What is required to schedule an appointment?

  • A physician's referral is required.  In addition, we will need an authorization from the work comp carrier, including the claim number for the open case. 

  • Please call us at 205-968-1616 to reserve an appointment for your client.

  • Physician's offices, Adjusters and Case Managers may either fax the necessary documentation to 949-561-4119 or email to scheduling@ 

What types of insurance do you accept?

  • At our clinic, we treat injured workers exclusively. 

  • We can bill either workers compensation carriers/work comp networks or self-insured employers upon execution of direct agreement.

How can I set up Post Offer Employment Tests or other Employer Services?

  • Please contact us by email at  or by phone at  205-968-1616  to discuss the process for initiating employer services.

  • A site visit and project evaluation are typically needed to provide a quote for services.
